CUUEG Expeditions - Participant Information Questionnaire

To efficiently organise the expedition, the Expedition Organiser will need certain details from you. The questions listed below apply to almost every trip. The Expedition Organiser will also let you know what other info is required from you - probably by email.

Please answer the first set of questions when booking onto or asking about booking onto a trip.

Please answer the second set of questions when asked, usually about a month before the trip leaves.

If any of the details change significantly (he does not need to know every time you do another dive!), please inform the Expedition Organiser.

If completing this file as an attachment (rather than cut/pasting into an email) please include your name so that if the organiser prints the answers (s)he knows whose answers they are!


First set of questions - to be answered at time of booking

1. What is your highest BSAC Qualification or SALT equivilent?

2. How many dives have you done in total?

3. How many dives were in the UK?

4. How many dives were since 1 Jan this year?

4a. If your answer to Q4 is less than 10, how many last year?

5. What was the date of your last dive?

6. Do you have the 25m depth progression signed off and/or experience in the 25m + range?

7. Do you have the 30m depth progression signed off and/or experience in the 30m + range?

8. Do you wish to travel from Cambridge? (If not, where from?)

9. What time can you leave on the day we leave?

10. Do you have (or have access to) a car, if so what type?

11. If asked, could you drive a van?

12. What is your date of birth?

13. What type of suit are you using (wet/dry)?


Second set of questions - to be answered approx one month pre-departure

15. Please confirm if you have the following SDCs (or direct equivilent)

a) O2 admin

b) FAD (first aid for divers)

c) PRM (practical rescue management)

d) DPM (dive planning & marshalling)

16. Please list other agency qualifications, or SDCs that you think important.

17. Which instructor(s) has/have seen you dive recently?

18. What gas(es) are you planning to dive?

19. Please give next of kin (NOK) details (Name, relation to self, phone number(s), address).

20. Please state what kit you need to hire (full details please ie, size of backplate, preferred lead size/quantity[ ]x2lb, [ ]x4lb. We will assume a preference for DIN over A-clamp unless stated otherwise. And don't forget items such as: Bag, D.Timer, DSMB+Reel, Weightbelt, Compass).

21. Please provide any relevant dietary info, medical allergies etc.

22. Please provide any other info you think is relevant.

23. Please provide updated responses to any questions from set 1 that have changed significantly.
